What do I need to bring?
Be prepared to provide your previous medical or surgical history. If you have any scans/MRI reports, please bring them along with a list of your medication. You may wish to bring shorts, and a t-shirt or sportswear.
How many visits will I need?
The number of treatments will vary depending on the treatment needed for each individual. After a thorough assessment, Dr. Bagley will discuss their treatment plan for the rehabilitation process.
Do you take insurance?
Patients can reach out to their insurance providers to see if we accept their insurance. We do not take insurance directly, but you may go through your insurance provider to receive reimbursement.

Do I have to be referred by a doctor?
No, physical therapists can assess and treat without referral by a doctor. However, some health insurers require a doctor's referral before they authorize cover of fees.
Treatment duration depends on your diagnosis, the severity of your impairments, your past medical history, and other factors. Re-evaluations will be done to assess your progress and your therapist communicates with your physician to help you achieve your therapy goals.

If you are looking for physical therapy covered by your health insurance provider, you can contact them to determine if outpatient physical therapy services are covered under your plan. We recommend that you confirm your with your provider that you may receive reimbursement. It is up to the patient to collect their reimbursement.
